Skilled Care
After a hospital stay, your loved one's physician may prescribe that they patient recuperate at a skilled nursing facility prior to going home. Cheltenham's Interdisciplinary team works closely with the physician to determine the appropriate course of treatment. The goal of the interdisciplinary team is to maximize your loved one's self-sufficiency and may include the following services:
Rehabilitation Services, IV therapy, Wound Care, Enteral Nutrition and Pain Management
Intermediate Care
Cheltenham provides 24-hour nursing care for residents who are no longer able to live independently. Your loved one's needs will determine the services to be provided. Available services include any or all of the following:
Dietary, Social Services, Activities of Daily Living, Therapy Services and Ancillary Services (Pharmacy, Lab and x-ray)
Respite Care
Cheltenham is available to provide you with peace of mind when the situation arises that you need assistance in caring for your loved one. Be it a business trip, vacation or just the need to get away for a few days, the professional staff is available to assist you in anyway possible.
Hospice Care
Cheltenham is available to assist you during this trying time. The professional staff will work with you to ensure that your loved one is as comfortable as possible utilizing the following services which are available:
Pain Management, Dietary Services and Enteral Feeding
CN&RC
At Cheltenham each resident is assessed and provided with a detailed care plan that will enable the professional staff to maximize the quality of your loved one's stay.
Cheltenham utilizes an Interdisciplinary team approach to determine the most appropriate plan of care for each resident. The "Team" consists of Nursing, Therapies, and Dietary Services, Activities and Social Services Departments.
We are available to admit new residents 7 days a week and 24 hours a day.
